Contact a new  attorney. Your agent or carrier should have helped you with the death if they requested Certificates. Now it's up to The Department of Insurance to aid and assist. I've read some comments here, going back over 20 years is doubtful.  

Always read the documents that are attached to the policy. This issue you're bringing up is a huge problem for many, And shame on your attorney who didn't advise you correctly either.....
