Ensures patients and their families, have access to a medical home (medical, dental and behavioral services).Provides initial intake for prenatal patient and follows care throughout pregnancy and after delivery.Facilitates appointments for well child, prenatal, immunizations, medical, dental, behavioral health, family planning, STI testing/treatment, etc. Engages patients in the development of health and social goals.Maintains contact with patients/families at least monthly until services are no longer needed. Provides pre-natal and parenting education on best practices, including breastfeeding, safe sleep, proper car seat installation, etc.
