About Us
Family Resource Specialist (FRS) at CHC/SEK are motivated, and passionate about improving health outcomes for pregnant patients of all ages and children/families. This position provides services for anyone experiencing pregnancy, children, or families need of care management.
This position completes initial interviews to determine patient and family needs, both health and social. FRS go above and beyond, often assisting patients and their families through innovative methods in nontraditional locations such as a patient’s home or in the community. FRSs must be knowledgeable about women’s health and family support, community resources, and CHC/SEK care systems. Their main goal is to address barriers to improved health outcomes.
