 From a Canadian perspective and if you're thinking about the same for the Canadian market, it is a bit more problematic -  you must be registered with a public plan provider (at the provincial level and through NIHB) to qualify for the private plans because one follows the other.  I hope this is helpful.

No you would need insurance contracts to be able to bill directly... I have a 10 person billing team if you need it...

I recommend researching the payer to see what their medical policy and guidelines state, since ever payer is different. You might be able to bill, but then the question is, will reimbursement be issued?
