One of the most important moments in a patient's journey is the transition from the hospital back home.
When healthcare professionals, caregivers, and community partners work together, patients often experience a smoother recovery and greater confidence during this transition.
From your experience, what do you believe is the most important factor in helping someone transition safely from the hospital to home?
