STANDARD Individual PLAN $50/mo and only $25 office visits or
PREMIER Individual PLAN for $125/mo with $0 office visits

(Family Plans include 2 Adults and up to 3 children)
STANDARD Family $135/mo
PREMIER Family $350/mo
