The Care Continuity Program is a controlled substance-specific, patient-reported outcome tool that is completed by the patient, outside the office, prior to every visit. The results are scored, summarized and automatically appended to the electronic health record. This program mitigates both the risk to the patient and provider by delivering clinical and medicolegal data prior to each prescribing and dispensing decision.
