I use Canadian Web Hosting out of Vancouver. They are very good. When I have problems with my stuff or a clients' (usually it is a client's website) They are very quick at diagnosing and fixing it and communicating through the process.

I have a dedicated machine with them, but under one of their other business names: http://www.hostingmetro.com. I have the managed plan, which means I take care of 99% of issues, but have back up support when I need it. Been with them for 20+ years. I host about 60 clients.
