Typically the human resources department will be in charge of this. If there is not human resource dept. than there should be some team that develops it. If you find that outsourcing is too much, have a small team brainstorm everything and Google online handbooks. There's a ton of free info on Google. Finally, as to how often, I guess that would depend on what goes on. I say yearly at a minimum unless a situation arises and there is no written way to address it.  Again, have the team meet and go over the situation, come to an agreement and add it to the policy. Go to a mirror and practice practice practice. Introduce yourself to one new person every day. If you have to make it more than an introction to relieve the fear, bring something that you can give them (besides a biz card) that gives you a reason to meet. Once you practice this often enough, you may begin to overcome the fear that comes with cold calling. Get your script down and practice it. Reviews are very important to raising your ranking online. You should ask for a review or testimonial every time you do something for someone. Just Ask! Depending upon your business, most people will be happy to leave a review for you. Ask while they are still happy from your service or their purchase. The best reviews are video reviews.
