(more) Medicaid spend down should be considered as a "monthly deductible," from a practical point of view.  For Medicare beneficiaries, there are options that could sharply reduce these potential out-of-pocket costs. For non-Medicare beneficiaries, this is also possible, but a privately-purchased health insurance policy would need to be purchased.  This is a relatively-common (and difficult) problem, since the person isn't quite eligible for full Medicaid benefits in a particular state.
