Go directly to your resources. Hospital case managers, Assisted Livings, nursing homes. Now with COVID-19 those resources might be closed for you, but as we open the country go to them. Also make friends with local churches, the folks there know who in the community needs help. There are also private Care Managers who help people privately in their homes with MD appointments and finances, they can refer to you for Home Services. I started a group 17 years ago we meet monthly to refer to each other, check it out www.wmepa.com. You can work with others to start your own group

No this is local to Western Massachusetts - It is all volunteer our members are from Nursing Homes, Assisted livings, Home Care, Businesses, hospitals etc They host the monthly meetings for a little breakfast we have around 40 people attend each month. We refer to each other most of the time
