I've outsourced both onshore and offshore and there are similarities and differences. In all cases communication in both directions is the key. Also you need to have reasonable expectations as remote workers aren't going to be as efficient as someone sitting next to you.

Timezones can be a real impediment. English skills and cultural differences are something to watch out for. Some cultures have a hard time saying relaying negative information to authority resulting in a project that appears to be going well until it totally implodes near the end.
I don't worry about the morality of the pay differences because it is a market and they are offering you a rate, you aren't forcing it on them. $4K annually is a pipe dream. I pay more around $4k monthly for intermediate/senior full time programmers. You can pay less, but you often get what you pay for.
The big thing most people miss the first time they outsource is that you will need to dedicate time to managing the remote employees, more time than you would normally dedicate to an onsite employee, potentially much more. This can often wipe out any cost savings. My assumption at this point is that I'm paying about 80% of US rates to have an outsourced team when I factor in the additional time spent by my staff to manage them. The primary reason I outsourced in the first place wasn't cost, but talent availability.

Also fiverr is really for well defined small tasks. If you need something larger it isn't the right place to look.
For financial transactions we require 2 people to sign off. This has caught fake invoices a number of times.
