Depending on the size of the business, referral to an internal, confidential resource such as Employee Assistance Program  is helpful.  Referral to a previously negotiated contracted professional, independent of the employer, who is fully licensed, screened and has office space.  These referrals are for "personal issues".

If the issues are about work, attitude, effective completion of assigned work, these are handled by the immediate supervisor, documented with expected corrections/instruction given to the staff member.  A follow up based on this meeting should be done at 30 days including documented progress or lack of same.  

Situations which involve legally protected classes, inappropriate work place behavior, accusations of employee mistreatment, harassment, reasonable accommodation are handled immediately without delay.

Clear documentation, dated and signed by parties to the workplace issues are required and filed in a confidential manner.
