As a Geriatric Care Manager with knowledge of aging issues I assist older adults and their families to achieve a continuity of care at various stages, thereby reducing the burden on families.
How We Got Started
I witnessed too many families struggle after a hospitalization only to return to the facility because they did not have needed resources and medical support either in the home or in the community.
